In this short manuscript, we discuss the role and present the practical aspects of a guidewire inserted distally in the ipsilateral arm in facilitating venous localization and access in the context of cardiac rhythm management device implantation, reducing the risks for complications, such as pneumo- and hemothorax. We believe that this notion may encourage practicing device-implanting physicians to keep this approach in mind and as a fallback strategy during their cases.
