The treatment of chronic lateral patellar instability caused by trochlear dysplasia can be challenging. When nonoperative treatment is unsuccessful, a sulcus-deepening trochleoplasty may be indicated. However, contraindications for a trochleoplasty include grade-IV chondromalacia of the patellofemoral joint. Even with this contraindication, an osteoarticular allograft transplant of the trochlear groove can serve as a salvage procedure to improve knee function.
