Evaluation of the completeness of information sources used to prepare the best possible medication histories at a tertiary teaching hospital in Jordan.

The results show that pharmacist's interview with the patients scored the highest percentage of completeness compared with hospital pharmacy database and medical file and is, therefore, considered more comprehensive in obtaining the BPMH.
