Endothelial dysfunction and vascular remodeling in PH secondary to pulmonary fibrosis enhance fibrogenesis through impaired BMPR2 signaling. Tacrolimus may have value as treatment of advanced IPF and concomitant PH. Genetic abnormalities may determine the development of PH in advanced IPF.
