We report a case of <i>Neisseria meningitidis</i> symptomatic proctitis in an HIV-negative man who has sex with men attending the genitourinary clinic in Malta. The proctitis was caused by a rare clinical unencapsulated penicillin-resistant <i>N. meningitidis</i> strain of the ST-53 clonal complex.
