Antitachycardia pacing (ATP) during charging, which delivers ATP for rapid ventricular tachycardia (VT) while charging for a shock of an implantable cardioverter defibrillator (ICD), has been used as an adjunctive therapy to terminate VT. We report a rare complication of ATP during charging, in which ventricular fibrillation was induced by inappropriate ATP during charging for rapid atrial fibrillation (AF) in a patient with hypertrophic cardiomyopathy. The present case suggests that AF increases not only the risk of inappropriate ICD shock, but also that of degeneration to VF due to the inappropriate ATP. This article is protected by copyright.
