A 51-year-old female Bulgarian migrant living with HIV presented with abdominal pain, cough, and elevated inflammation parameters. Computed tomography revealed extensive cystic structures perforating from the liver through the diaphragm into the right lung, where they showed bronchial communication. The finding corresponded to a transitional cyst stage of cystic echinococcosis.
