Tacrolimus clearance increases during pregnancy, resulting in decreased exposure to tacrolimus, which is explained by gestational age and haematocrit. To maintain pre-pregnancy target whole-blood tacrolimus pre-dose concentrations during pregnancy, increasing the dose is required.
