Minor ABO incompatible LuTX recipients are at considerable risk of developing clinically significant PLS. Post-transplant monitoring combining red cell serology and hemolysis marker determination appears advisable not to overlook hemolytic episodes which necessitate antigen-negative transfusion therapy.
