Assessment of prescribing pattern and adverse drug reaction in patients receiving anticoagulant therapy: A prospective observational study.

Heparin was found to be the most commonly used anticoagulant, Combination therapy of antiplatelets and antihypertensives was observed in most of the prescriptions, which is more effective than a single therapy. Polypharmacy was encountered in the study with an average number of 11.7 drugs per prescription. Several anticoagulant-related drug interactions were identified and there were two adverse drug reactions with Heparin. The prescribing pattern can be improved by reducing the number of drugs prescribed.
