Ultrashort-segment Hirschsprung's disease USHD is a rare entity usually diagnosed in male patients within the first year of life. We report the case of a 25 year old man with a previous colorectal anastomosis at 6 months of age and a lifelong history of persistent constipation. Massive megacolon was demonstrated on abdominal CT scan. USHD was suspected and anorectal sphincter myomectomy was planned as a diagnostic and therapeutic procedure (video S1).
