Lung allografts that develop PGD have a microbiome enriched in anaerobic oropharyngeal taxa, elevated gastric pepsin, and hyperinflammatory phenotype. These findings suggest a possible role for peri-transplant aspiration in PGD, a potentially actionable mechanism that warrants further investigation.
